Are you eligible?
The following criteria needs to be met to access my services
· Adult (over 18)
· Physical/intellectual disability or frail aged
· DVA or NDIS funding (plan or self-managed only) or able to pay privately
· Live within 2 hours of Toowoomba
Assessment
This will look different for everyone but will usually involve
· Interview/conversation with you
· Observation of home environment and tasks performed
· Assessments completed in the home
· Liaison with others who know you well
Recommendations/Intervention
This may include
· Home/office (ergonomic) modifications
· Assistive Technology trials/setup
· Manual handling training
· Falls prevention education
· Cognitive impairment/dementia advice
· Pressure area prevention/management
· Activities of daily living capacity building/retraining
· Report writing/Funding applications
